Misprint on UCSF Antibiotic Dosing Card for Zosyn in Hemodialysis

There is an error on the most recent (5/2007) version of the UCSF Antibiotic Dosing Card.  In the last column, under "Dialysis (HD or CRRT)" the hemodialysis dose is given as 2.25g IV Q6h.  The appropriate dose is 2.25g IV Q8h.  The dosage in CRRT is correct.

Please note this change (the card can be marked with a pen).  The inpatient pharmacy and ID pharmacy will be monitoring Zosyn dosing.  Note that 2.25g IV Q6h is never a recommended dosing schedule for Zosyn.
